New Player Registrations

This year all new players to the club must have a physical and clearance from the school before Tuesday's evaluation. The university will provide a free physical today (8/30) and will submit that paperwork to the school for you. It is between 5 and 7 pm at the Sport Medicine Institute located at 160 Farber Hall on south campus. I highly recommend that you make every effort to attend this physical. However, if you can’t make the physical the link below to the new player physical form must be filled out by your physician and submitted into the school before Tuesday. If you choose this option you must submit your paperwork to the SA office (350 Student Union) prior to Tuesday so they can process it for you. Remember Monday is a holiday so try to attend the physical.
