Prospective and Current players

For the upcoming 2009-2010 season all new players to the Men’s Ice Hockey club team must have a physical before on ice evaluation.

The University will provide a free physical.

Contact Sport Medicine Institute located at 160 Farber Hall on south campus. (716-829-2070) for an appointment.

Returning players are encouraged to obtain a new physical before the start of the season as well.

If you reside outside the Buffalo NY area or would prefer to see your own physician, the link below to the player physical form “Club Sports History” patient forms is provided.

This form must be filled out by your physician.

Retain the completed signed physician form and bring to the first on ice evaluation.

You will not be able to participate in the on ice evaluation
without a signed compleated form.

There will be no exceptions.
